AMS nurses initiate and begin monitoring anticoagulation at hospital discharge, helping get patients to therapeutic range, and transitioning them into the long-term maintenance program.

Target

Targets patients discharged from MGH who are prescribed warfarin.  The vast majority of the patients are over 65.
