Why

“The Nursing Model for Anticoagulation Management was implemented to provide improved continuity of care to patients as AMS takes a more comprehensive role in anticoagulant management for patients across the care continuum. Previously, care was fragmented and task oriented. Patients often received care from several nurses during an episode of care, which did not consider the concept of knowing the patient. By treating patients as unique individuals the Nursing Model for Anticoagulation Management has a significant impact on therapeutic decision-making, allowing this important value in nursing to be actualized.”
