Involving patients and families from the beginning planning stages was crucial to developing a model that is responsive to patient needs.  High Point continues to incorporate patient and family feedback gained from listening and learning activities, discharge follow-up calls, daily patient rounding process, and focus group surveys.  Feedback is utilized to refine the model, evidence based protocols, and patient education materials as needed.

Focus

When High Point leaders began the process of looking for a new care delivery model, they found models that focused on how best to organize and manage nurses.  But what they wanted was a model that focused on the patient and organizing care planning, delivery, and education for the patient’s needs.

Continually

The Collaborative Patient Care Management model is flexible, and the team-based nature of the model allows continued innovation.

Early

Involving finance from the beginning with the initial review of the model helped create a strong business case and foundation for the model.  This also helped High Point think through necessary reports to track the impact of the model on a routine basis.

Community

The Collaborative Patient Care Management model is actively involved in community outreach, disease management, and health promotion activities including the provision of the following programs:  outpatient smoking cessation, prenatal education, diabetes education, heart failure education, Concerning Women program, and menopausal education.  A variety of active support groups and bi-annual health fairs and community screenings promotes community health awareness.
