Focus

The ability to understand the unique and complex needs of those with long-term or advanced illness requires a commitment by a health professional to bond with the patient. This focus is not generally taught at the academic level nor often experienced prior to employment with Evercare.  Evercare providers must learn this and incorporate this into the way they practice the art and skill of care management.

ImportanceReimbursement

Evercare is primarily reimbursed through a capitated, risk-adjusted model which allows for greater flexibility in ensuring that each individual enrolled in their heath plan will receive the type of service or intervention they need.  Capitation allows for the up-front funding for NPs and CMs to help facilitate care and the creation of extra benefits such as preventive dental care, eyewear, access to medical products and equipment, and even home modifications.
