Utilization

A Medicare-funded study published in the October 2003 Journal of the American Geriatrics Society of 3,653 nursing home residents, half of whom were managed by Evercare, found lower rates of hospital utilization for Evercare patients. Evercare patients averaged 2.4 hospital admissions per 100 residents, compared to 4.6 for standard nursing home patients. For those patients who were hospitalized, Evercare patients average 4.6 days in the hospital compared to 5.4 for standard nursing home patients. 1

According to the same Medicare-funded study published in the October 2003 Journal of the American Geriatrics Society mentioned above, the Evercare approach and continuity of care has been shown to help prevent some problems before they become serious — reducing hospitalizations for nursing home residents by 45 percent and trips to the emergency room by 50 percent. 2

The study estimated hospital cost savings per nurse practitioner per year of $103,000. 3

The Diversion Project in Florida, in which Evercare participates, resulted in a 70 percent reduction in nursing home placements over four years. In addition Evercare Select operates under the Arizona Long Term Care System, a Medicaid-funded program that delivers integrated, quality long-term care to Arizonans who are older or have disabilities and are in need of skilled care. As of June 2003, over 60 percent of elderly and individuals with disabilities in Arizona were able to avoid nursing homes and remain in their communities compared to 5 percent at the program’s inception.

Patient

As discussed in the above section, Evercare has demonstrated a reduction in hospitalizations and ER utilization with no change in or slightly improved mortality rates.

Provider

  • In a 2006 survey, 92 percent of nursing home staff experienced overall satisfaction with Evercare. 4
  • In a 2006 survey, 91 percent of physicians are likely to continue working with Evercare. 5
  • In a 2006 survey, 92 percent of nursing home administrators report overall satisfaction with Evercare NPs, and 92 percent report satisfaction with NPs' relationships and communication with physicians. 6
  • Anecdotally, NPs and Care Managers are very satisfied with working at Evercare. They enjoy the ability to build intimate relationships with their patients and really manage their care, and they truly feel the difference they are making. In addition, they like the flexibility of working from home.

Patient

  • 98 percent of families of nursing home residents enrolled in Evercare are satisfied with Evercare’s compassionate, personalized care. 7
  • In a 2007 survey, 97 percent of patient families of Evercare members in nursing homes rate Evercare nurse practitioners as good or excellent. 8
  • In a 2007 survey, 88 percent of members and responsible parties said they were satisfied with Evercare’s Medicaid plans. 9
  • In a 2007 survey, 94 percent of Medicaid plan members planned to continue their membership with Evercare. 10
