Home Healthcare Telemedicine began with CHF patients but has expanded to include COPD patients.  The average age of patients served by this model is 76.

Organizational

The model is based out of a Medicare-licensed home care agency, Presbyterian Home Healthcare, which is owned and operated by Presbyterian Healthcare Services, a not-for-profit integrated delivery system that includes seven hospitals, a health plan, and a medical group.  The model serves patients located throughout New Mexico, in both urban and rural areas.

Presbyterian Home Healthcare in Albuquerque is a department of the Central New Mexico Presbyterian hospital.

Intended

Home Healthcare Telemedicine is intended to serve chronic care or hospice patients in their homes.  The model could be run by a home health care agency, a hospital, a health plan, or other integrated healthcare delivery systems.
