Utilization

Prior to starting the program, CHF patient re-hospitalization was already low (approx 6%) due to a preexisting care management program.  After implementation of Home Healthcare Telemedicine, CHF re-hospitalization rates dropped to close to one percent.  In addition, the overall CHF hospitalization rate for 2007 is low, at 12.2%.

The return on investment for the machines has been easy to demonstrate as one prevented hospital admission pays for 1 unit ($5500).  In addition, using the technology drastically reduces nurse time, as there is significant travel time for home health nurses in their regional coverage area.

Telehealth RN productivity is almost double visiting nurse productivity.  On average, visiting nurses conduct five patient visits every eight hours compare to telehealth nurse case managers who conduct eight visits every eight hours.

Presbyterian is in the 90th percentile of national rates for prevention of acute care hospitalizations overall.

As stated in the above section, the Home Healthcare Telemedicine model has helped reduce readmission rates for CHF patients.

Patient satisfaction with the telemedicine program is 96 percent.  The measurement focus is "Likelihood I would use Telemedicine again if my condition indicated it, or if recommended for me."
