Implementation

UPMC Shadyside implemented the Primary Care Coordinator in 2002.  The model took four months to implement and is 100 percent implemented on all units of hospital. The model was refreshed in 2006.

Recruitment

New hires are screened with a formal Performance Based Development System (PBDS) to assess critical thinking prior to hire.  Clinical Nurse Leaders are prime PCC candidates.

Training

UPMC Shadyside has developed a specific orientation program for the Primary Care Coordinator Role.  All staff participate in the program, and the program focuses on patient centeredness as a core value. 

PCCs are encouraged to pursue Masters degrees.

Replication

UPMC St. Margaret implemented the Primary Care Coordinator model successfully in 2003, and they have achieved similar results in ALOS and care coordination as UPMC Shadyside.

UPMC Braddock is implementing the model in 2008.

Considerations

The Primary Care Coordinator model can be widely replicated in acute care hospitals.
