The model supplements the primary nurse caregiver with additional nurse and allied health support and equips caregivers with comprehensive online care planning and documentation. 

General inpatient population.

Prairie Lakes Healthcare System is an 81-bed, full-service, not-for-profit community hospital.  Prairie Lakes is located in the rural community of Watertown, South Dakota (population 20,000), which is 100 miles away from tertiary referral centers.

Acute care hospitals.  While created and initially implemented in a rural community hospital, this model is applicable to all acute care hospitals.
